Sim disappeared instantly instead of dying

My Sim was due to die of old age (110 days old!) and was just going about her day when she suddenly disappeared, and an urn instantly appeared on the floor. Nobody reacted, not even her husband has any moodlets about it. I think possibly the death animations failed to play? Please help!

EDIT: Forgot to mention that there was no appearance of the Grim Reaper either.

Never had this i would have to kill one my sims after i save and then try That's the only way to see it this a new glitch in the game.
